The "30 Baht Scheme" a key achievement of recent health care reform in Thailand, provides universal health care coverage for Thai citizens. This policy has I,portant ramifications for the practice of psychiatric mental health nursing. Limitations in the the time allowed for hospitalization may significantly restrict current nursing practice and the ability to implement prescribed nursing care plans. Therefore, it is important to understand the implications of the 30 Baht scheme on the management of nursing care for individuals with mental illness. The purpose of this study ws to answer the following two research questions: 1) What are the effects of the 30 Baht scheme on mental health and psychiatric nursing care menagement?, and 2) What are the problems and obstacles to providing mental health nursing management as a result of 30 baht scheme?This study utilize a case study approach to examine the effects of the 30 Bhat scheme on mental health nursing management at apsychiatric hospital in Northeastern, thailand. Purposive sampling was used to select seven participants who are professional nurses and who have worked on an inpatient psychiatric ward under the 30 Baht scheme for at least 1 year. Data were collected using a semi-structured interview developed by the researcher. Content analysis of interview data was used to examine the effects of the 30 baht scheme on psychiatric mental health nursing management. Approval for this study was obtained from the Ethics Committee, Faculty of Nursing‹ Chiang Mai University and informed consent was obtained from all participants. Content analysis of interview data revealed five themes of mental health nursing management formed as a result of the 30 Baht scheme: 1) individualized routine hospital care; 2) rearranged activities; 3) establishment of health care service networks in the community: 4) complexity in coordinating nursing management; and 5) problems and obstacles of mental nursing management. Findings from this study suggested a greater emphasis on mental health nursing management in the community as a result of the 30 Baht scheme. These findings are relevant for the development of community approaches to mental health nursing management. Mental health nurses should take a greater role in the management of individuals with mental illness in the community, preparing them to live successfully after discharge from the hospital. This continuity of care can be accomplished by transitional care services integrating hospital and community mental health services to support the transition from asylum to community. This approach will have a significant impact on meeting the changing mental health needs of clients, familles, and communities.
