A 30 years-old Muslim woman an employee came to the psychiatric unit at Yala regional hospital on December 2005. She has had the palpitation and hypervigilance for about 2 weeks. She also has had chest discomfort and felt not enough in breathing difficulty in sleep easily to wake up at night and take a long time to go back to sleep again. She always has had bad dreams and frequently awaken. She said she could not concentrate on her work and had a little confidence in many activities. She was very sensitive to loud noise and darkness .She always feel scared like her life is in threaten situation all the time frequently felt afraid and tremble as soon as she were in the terrible situation. Six months ago she had a terrible experience exposed the terrorism event while she was walking along the street the main power of the city went off. Few minutes later there were shooting and bombing police station and many place in the town. She decided to run to a bookshop and hided under the desk listening to gun firing and bombing for a long while. She thought she might not make it and died. Afterwards she had insomnia and frightening for about 2 weeks. Two years ago she had been diagnosed having panic disorder with agoraphobia. She had medical treatment by psychiatrists. She had alprazolam and exposure therapy for about 6-9 months. One year later the symptoms of panic attack and phobias were gradually disappeared. The mental status examination has shown that she looked anxious relevant speech appropriate affect and behavior no sign and symptoms of psychosis. The physical examination and routine laboratory investigation were in normal range. After receiving SSRI for 3 months and having short term benzodiazepine plus breathing exercise the physical symptoms were gradually improved but she still had inappropriate worry anticipatory anxiety about violence event hypervigilance less concentration in daily activities sometime she was depressed and had low self esteems. Therapists had used religious concept on the ground of patient’s bearing process and intervention e.g. The pillar in Islam praying activities (Solat) blessing to the God (Do-a) concept thinking about to be tested etc to cure this patient. That cognitive and spiritual concept together with medication had improved all signs and symptoms of this patient.
