Depressive disorders are commonly found among patients at primary care clinics. They are often chronic or even severe depressive symptom may not be identified early and treated as most patients present with somatic symptoms instead of depression. Further sleep disturbance is often a predictor of depressive disorders. Screening for depressive disorders in primary care clinic can lead to appropriate intervention and prevention of chronic and severe disability. Purpose: The purpose of this study is to detect depressive disorders in patients with sleep disturbance. The study was conducted from January to December 2005. Population comprised of 72 sleep disturbance patients of more than 1 year from Baan Meung Wa Village Health Clinic, Sansai City, Chiang Mai Province. Research instruments included a recording demographic data form and health and screening instrument (KKU-DI) for depressive disorders. Reliability of KKU-DI was obtained by Cronbach’s alpha coefficient and was 0.91 Descriptive statistics was used for data analysis. Results: Results revealed that of 72 the patients who had sleep disturbance 41 had depressive disorder (56.9%) in which 24.4% were male and 75.6% were female age range form 21 to 60 years old and age average of 45 years old (SD=9.7). Of these 15 patients (36.6%) were referred to a state mental hospital for diagnosis and treatment. Their psychiatric diagnosis were as follows: Severe depressive episode without psychotic symptoms (6 patients 40.0%), Moderate depressive episode (1 patient 16.7%), Dysthymia (3 patients 19.9%), Adjustment disorders (1 patient 16.7%) Panic disorders (1 patient 1 6.7%), Mental and behavioral disorders due to use of alcohol (1 patient 6.7%), and other depressive episodes (2 patient 13.3%). Conclusion: The results of this study suggests that depressive disorders could be found in patients who receive primary care clinic with a symptom of sleep disturbance. Therefore community psychiatric nurses should be aware of this in order to provide early detection of depressive disorders. Moreover so patients can receive appropriate diagnosis and treatment to avoid chronic or severe depressive disorders.
