This study was aimed to assess policy implementation on establishment of primay care unit and its performance at provincial and district levels in the second pilot project of the Universal coverage scheme. Four provinces of the first and second wave policy implementation, Samut Sakhon, Phuket, Sukhothai and Ubon Ratchathani were purposively selected based on the financial model adopted. Health managers, health personnel, community leaders and people in the four provinces were interviewed in-depth and discussed in focus group's gathering. The study was carried out during November 2001-April 2002. Service provision model adopted by those primany care units did not fit into the family medicine concept due to lact of appropriate manpower and misleading conceptof some managers of contracting units of primary care. Publice sector participated on health matter at a low level, however, they reaped the benefits from primary units of the hospitals, in terms of easily access and better quality of care received, especially among those elderly and those with chronic diseases. For promotive and preventive care provision, the main problems were lack of clear operation guidelines and capitation payment. All budget was directly allocated to contracting units for primary care even though some of them showed no potential. Furthermore public health personnel were also very concerned with the new per capita budgeting which covered their salaries and the imbalance the work load in large districts. The existing primary care unit were in a transition period and adapting themselves to abide by the provincial regulations and be ready for further evaluation rather than to intentionally promote public health. The intensive man power and structure reform are needed for successful implementation.
