Objectives: To examine the characteristics of patients with committed suicide and the circumstances of committed suicide in Suan Prung Psychiatric Hospital. Methods: We reviewed the medical records between year 1995 and 2004 of all inpatients with committed suicide of Suan Prung Psychiatric Hospital. The staffs directly responsible to the incidents, such as physicians, head-ward nurses, and in-charge nurses were interviewed. Results: During the study, 10 inpatients (9 males and 1 female) had committed suicide in the hospital. Most of them were 21-30 years of age, had a chief compliant of psychotic symptoms, and had a diagnosis of psychotic disorders (7 cases). Only 3 cases had a history of suicidal attempt before the admission. In the following assessment, doctors and nurse could identify suicidal ideation in 3 and 4 cases, respectively. Methods of committed suicide were hanging with cloth, tube pipe, sweater bed sheet and restrain cloth (7 cases) drowning 2 cases, and jumping from a high building 1 cases. The events usually occurred during the day. The general opinions of the staff by the interviewing were that suicide attempts were hard to be predicted, the seclusion rooms were not safe enough, the restrooms were the high risk area and the staffs were less aware of sociality when patients were in recovery, etc. After the events, the staffs were more aware of suicide assessment, increasingly used of emergency treatment including ECT, and modified the structure of some building. The in-charge nurses usually had negative feelings such as anxious guilt being criticized, and being blamed. Conclusion: Committed suicide in psychiatric hospital should be an issue of concern, The results of this study may be helpful for improving strategies for preventing committed suicide in psychiatric hospitals.
