The purpose of this report was to study the payment mechanism and budget allocation for the Universal Health Care Coverage Project at Lamlukka Hospital from October, 1, 2001 to September, 30, 2002. The data was retrieved and analyzed from Lamlukka Hospital database. It was found that a point-based payment system allowed the hospital to reimburse 197.31 baht per point for health promotion and disease prevention activities and 52.93 baht per point for outpatient service. The relative weight per unit for diagnosis related groups reimbursement was 2,932.78 baht. Thus, the total budget allocated to Lamlukka Hospital for the Universal Health care coverage project during the period was 10,723,086.23 baht, or 251.69 baht per universal health care card. The point-base payment system for health promotion and disease prevention activities and outpatient service eased the cost control, simplified the administrative processes, and moderately enhanced quality of the service. It encouraged health promotion rather than disease treatment activities as more reimbursement per service point was allowed for health promotion and disease prevention activities and outpatient service. The diagnosis-related group payment mechanism under global budget for in-patient service easily accommodate case control. However, the administrative difficulty had moderate impact on quality of care. The budget allocation system worked well and should be considered to apply to other community hospital participating in Universal Health care coverage providing that outpatient and inpatients service rate were taken into account.
