Generally, psychiatric treatment in hysterical neurosis, conversion type focuses at changing and improving the unhealthy intrapsychic structure and pathological defense mechanism through psychotherapy. Behavior therapy, environmental manipulation, milieu therapy, family therapy or use of minor tranquilizer are other modalities, which often used alone or in conjunction with psychotherapy in the treatment of this illness. Higher incidence of hysterical neurosis occurs in young adults with frequent basic personality of immature and dependent type. In Thai culture, such patients are usually still living with their parents and under the care of the parents or other senior persons in the family. Therefore these people play an important role in the patients’ treatment. In this report the author described a case of hysterical neurosis conversion type whose symptoms were manifested by tremor of hands, legs, trunk and ataxic gait, by using insight oriented psychotherapy and collaborative family treatment by the psychiatrist and psychiatric social worker. The patient improved satisfactorily in 32 visits (23 months treatment duration). Significant improvement in the reported case was due to the change in the family psychopathology. The report advocated the importance of family therapy as a collaborative treatment in the case of hysterical neurosis, conversion type.
