Discharge planning for hospitalized elderly is the nursing process that crucial to the quality of life. It is for the patients not only admitted in hospital but discharged from the hospital as well. The elder patients usually have many problems that involved several organs, risk for medical and iatrogenic complications and very difficult to recovery. Furthermore, elder patients are being discharge from hospital “Quicker and Sicker” Thus, they are high risk for poor post discharge outcomes as well as progressive functional disability that result in caregiver dependent. The comprehensive discharge planning for hospitalized elderly is a systematic integrated multidisciplinary process focusing on patient holism. Nurses should assess strength and weakness of elder patients in general health status, functional status, mental and emotional status, perception of health and spiritual as well as assess appropriated sources for support the elder patients and their caregivers. Discharge panning is an ongoing process that begin as soon as the elder patients are admitted or within 24 hours after the admission. The elder patients and caregivers are continuously contacted by nurses during the process. The consistency evaluation and follow up are used for the discharge planning during the elder patients are admitted till they are discharged to their homes or communities.
