To study the clinical courses and risk factors for the relapse of methamphetamine psychosis during 6-month follow-up period. This multi-center study was prospectively carried out in 8 psychiatric hospitals. The participants were methamphetamine psychotic patients who received 4-week inpatient treatment and voluntarily accepted to receive additional 6-month outpatient treatment. On each monthly visit, Psychotic symptoms, global mental health, and methamphetamine use were assessed by the use of the Brief Psychiatric Rating Scale (BPRS), the clinical Global Impression (CGI) scale, and urine test for methamphetamine. For those who lost to follow up, telephone interview or questionnaire mailing were used to assess the relapse of psychotic symptoms, methamphetamine use, other substance use, psychosocial stressors, and antipsychotic treatment. Ninety-seven participants were assessed at least once during 6-month follow-up period. The clinical courses during this period were classified as follows: 1) psychosis remission for those who had no psychotic symptoms; 2) psychosis relapse for those who had no psychotic symptoms at discharge but had psychotic relapses during 6-month follow-up ; and 3) psychosis persistence for those who continuously had psychotic symptoms. Forty patients had psychotic relapses, the reuse of methamphetamine was found to be the most significant risk factor (hazard ration = 2.90, 95% CI= 1.46-5.79). Despite no significances, other substances use tend to be risk factor. Psychosocial stressors and antipsychotic treatment were not associated with psychotic relapses.
