This study aimed at reviewing international experiences of health care decentralization, describing roles and responsibility in health, and proposing options for health care decentralization in Thailand. The study methods consisted of literature review,qualitative, and quatitative,and quantitative studies. The study found that local government has a wide range of roles in health. Local government in developing countries faced two important problems in health care decentralization, financial constraint and health manpower management. Local government in Thailand has limited roles in health care provision and put health in the lowest priority. The revenue of local government revenue comes from two important sources consisting for various forms of local tax and central grant, which barely cover the overhead and utilities bills. There are few problems on service provision in the overlaped and common land areas. Inequity and cross boundary service utilization are major problems. Health survey data is very important for health care decentralization in Thailand because there is incomplete information. By considering financial factor, two options emerge for health care decentralization in Thailand. But, besides financial criterion, other factors, such as quality and efficiency of health care provision etc., need to be taken into account for appraisal of options for health decentralization.
