Objective To study the continual use of neuroleptic drugs alongwith lithium during continuation phase of treatment of mania. Method The author reviewed and analysed the records of 60 consecutive outpatients who were discharged from the inpatient division of Somdej Chaopraya Hospital during June 1 to December 31 , 1994 . The diagnosis included in this study were bipolar disorder,manic or mixed episode and manic episode according to ICD-10. Results At discharge, patients were receiving a mean of 603.25 mg/day (SD = 476.73) of chlorpromazine equivalents; 6 months later they were receiving a mean of 352.92 mg/day (SD = 365.05). This decrement was statistically significant. During this 6-months follow up period, 41 patients (68.33%) required no rehospitalization and 19 patients (31.67%) were rehospitalized. There was statistically significant neuroleptic dosage tapering in patient who were not rehospitalized but no statistically significant tapering in patients who were rehospitalized. Conclusions The combination of neuroleptics and lithium is an effective somatic treatment for acute mania. After acute manic symptoms have resolved, appropriate tapering of neuroleptics is suggested.
