Objective To study the characteristic of feelings guilt in relation to the severity of depressed mood in major depressive disorder, schizophrenia, anxiety disorders and normal controls. Method The study was conducted at the psychiatric outpatient department, Ramathibodi Hospital. Fifty subjects in each diagnostic category were studied. Normal controls were recruited from patients’ relative who revealed no psychiatric disorder from the interview. Depressive state was measured using the Thai Depression Inventory. Feelings of guilt was evaluated by a scale adapted from Berrios’s Guilt Scale. Result The reliability of the Guilt Scale was satisfactory with Cronbach’s alpha of 0.87. Exploratory factor analysis yielded two factors- ‘present guilt’ and ‘karma/sin’. Guilt was found in every diagnostic categories and was significantly correlated with depressive severity with highest level in major depressive patients. Conclusion Severity of feelings of guilt was correlated with level of depressed mood rather than with the diagnosis. The result suggested that guilt be regarded as a feeling lies along a continuum of depressive severity rather than as a present-absent dichotomy.
