Thailand has been on health care reform. The current health care reform has focused on community participation on health promotion in individual, family and community levels. The department of mental health also supports this policy by developing mental health service reform. Mental illness, especially schizophrenia and depression are burden chronic illnesses that require continuing community care. Currently many researchers and clinicians have been developing models of community mental illness care. Case management is one of mental health service system that has been successfully operated for helping persons with severe mental illness in the community. Case management is a systemic process of assessment , service coordination, referral monitoring, and evaluation through which the unique needs of clients are met. It works within a multidisciplinary team. This paper aims to present concept of case management and how to adapt case management within Thai context.
