This study utilized the “cost-of-illness” methodology (CO) to assess the overall negative economic impact of alcohol abuse on Thai society in 2006. Only tangible costs were counted in the study. Four main components of costs included in the estimation namely ; health care cost, loss of productivity cost, criminal and justice cost, and cost of property damage due to road traffic accident. To estimate health care cost, alcohol-attributable fractions (AAFs) for each alcohol-related disease were calculated for Thai setting using pooled relative risk from meta-analysis and prevalence of alcohol abuse in Thailand. Health care cost of alcohol drinking were, further, be estimated by multiplying the number of patients in each disease category that is attributable from alcohol drinking with cost per RW (Relative weight) for inpatient or the average expense for outpatients, provided by the national health authorities. For productivity loss due to premature mortality, human capital approach was employed. The present value costs of mortality were discounted at the rates of 3% and 6% for sensitivity analysis. To estimate the cost of work impairment due to alcohol, a cross-sectional household survey was carried out throughout the country. Thai populations from 4,330 households, who were sub-sample of household socioeconomic survey by the National Statistics Office in July 2007, whose age between 15-60 years and who were on paid employment, were interviewed using the Work Productivity and Activity Impairment (WPAI) format. Costs of criminal justice and law enforcement were calculated as the product of the unit costs and the number of alcohol-related criminal cases recorded at the courts and police stations. The proportion of alcohol-related cases was derived from the previous published data. Macro-costing technique was employed to calculate the unit costs. Cost of property damage due to road traffic accident was calculated by multiplying the number of accident caused by alcohol by average value of property loss for each crash derived from Department of Insurance, Ministry of Commerce. Similar to the finding from previous literature, the results of this study indicated that alcohol drinking was associated with a considerable economical impact, especially on healthcare and productivity lost.
