Objective: This paper describes the care strategies implemented for 147 patients who were case managed in the psycho geriatric unit and their clinical outcomes. Case Management is described as the “coordination, integration and allocation of individualized care within Iimited resources”. In the Institute of Mental Health, case management was introduced in 2000 beginning with the Early Psychosis Intervention Program and the Community Addiction Management Program. The psycho geriatric unit recruited case managers in 2006. Methods: Data mining of patients’ case managed from June 2007 to December 2007 was done. Analysis, using Microsoft Excel and Microsoft Access, was carried out to generate the quantitative results. Results: 82.1% of patients were Chinese, 6.2% Malay, 10.3% Indian and 1.4% others, 54.4% of patients were diagnosed with Dementia. Care strategies included 209 in dividual counseling sessions, 111 family counseling sessions, 212 telephonic case management calls and 352 referrals to other services, namely occupational therapist, physiotherapist, psychologist, diabetic educator and dietitian. With these plans implemented, clinical outcomes recorded 4.1% unplanned readmission within 28 days and 8.3% after 28 days. There were no suicides, incidences requiring crisis intervention or police intervention for admission. Conclusion: Implementation of care strategies, namely, psychoeducation, family-counseling sessions, Iinkages to services and telephonic care management resulted in positive clinical outcomes for patients. Therefore case management is a powerful quality tool that can be utilized in the promotion of quality care for patients.
