The aim of this paper is to review the emergence of community mental health care in Sri Lanka and discuss the challenges facing the future organization of mental health services. Information and data have been gathered from the past publications and the statements implicit in other documentations and practices on community mental health. There had been reference to community mental health since 1940s and since then the concept has evolved. Initially the practice was based on mental hygiene movement and child guidance services. Subsequently the concept was more on managing the long term mentally ill in the community and providing mental health services in general hospitals for mentally ill. In the 70s and 80s the establishment of day hospitals and community mental health centre extended this service for mentally ill. Since then there had been considerable discussion and attempts to develop a formal community net work of services as in the United Kingdom by making used of the primary health care net work. The idea was to make use of the existing primary health care team to recognize mentally ill, refer to specialized services when necessary and follow up in the community. These have not been largely successful due to a number of factors. In the recent years with the emergence of mental health promotion there had been at tempts to set up prevention and promotion programs through the primary care team. This paper reviews and critically analyzes these developments with the idea of proposing a more comprehensive mental health care program through community mental health.
