Addiction is regarded as a fundamental public health problem for Thailand. The problem appears to affect social, economic and to be obstacle for developing our country. The incidences of substance-related problems have continually increased. The government concerned about these problems, consequently Prime Minister’s statement all health professional and personnel are required to recognize those drug-addicted people as patients. They need appropriate physical and psychosocial treatment. Certainly, most of the drug workforces and dependent drug users realized that to achieve enduring abstinence from drugs without high motivation was not easy. It was comprehensible that majority of them relapsed in high rate after treatment. Not only drug abuser themselves, but also the others faced with high number of consequences. The prevalence of blood borne virus infection; for example HIV, hepatitis B and C viruses were high among injecting drug users. In Thailand, the common of illicit drug injection is heroin causing strong dependence and resulting in high relapse rate. Patients were treated by long-acting, orally administered opioid; such as methadone and buprenorphrine (sublingual), instead of heroin for detoxification or long term maintenance. There was compelling evidence that methadone and buprenorphine treatment reduced heroin use, the risk of HIV infection, crime and also a considerable improvement in social functioning among injecting drug users.
