In Nakon Sawan Province,suicide is an important problem resulting from stress. This chronic disorder may finally lead to success suicide. Surveillance is an important tool to prevent and help suicidal risk persons to cope with the disorder. This program has been implemented since 2001 yet with limited success. In order to develop surveillance model for suicidal risk persons by community participation, this action research was carried out covering span of 2 phases from June 2006 to February 2007 The first phase was to analyze and develop tool for effective surveillance through innovation and knowledge management process focusing on stimulation and motivating of 5 high suicidal risk districts (Kao Liao, Tak Fa, Takhli,Krok Phra and Phaisali) . The second phase was to fully apply the developed tool for actual surveillance in 1 tampons of the 5 districts by capacity building and empowerment of community leaders such as public health volunteers, administrators, members of local administration and health leaders. Its outcomes, were therefore measured in terms of surveillance participation and inter-exchange of knowledge. The evaluation of the impact of this study emerged through the results of surveillance and suicidal rate, comparing to that of the previous year, The ongoing surveillance was reportedly very able to screen, assess and follow up the high risk groups without any trace of committed suicide. As such, a great reduction of suicide rate related to that of previous year was observed. The important factors for development of surveillance model were good support of particularly strong network, participation and knowledge management which effectively combined to critically enhance local administration in becoming more aware and actively taking more control financially and in actual surveillance.
