Major mental illnesses are relapsing in nature and may even result in higher incidence of suicide and homicide. Psychiatric patients who absconded from the hospital might poses danger to self, family and society. Studies have that factors contributing to hogher absconding rate include poor security, poor monitoring, "unhappy patients' and suboptimal treatment. Study conducted in Hospital Permai Johor Bahru from 2001 to 2004 showed rate of patients absconded from male psychiatric wards were between 4.6% to 7.6%. This study proposes to explore the possible causes contribute to higher absconding rate and to evaluate ways to improve quality of care. Comprehensive management of psychiatric service can reduce the absconding rate. The data were collected from 1st January 2006 to 31th August 2006. Some of the factors which contributed to the absconding of patients were identified. The results showed patients who absconded from wards were those working in the workshop and were mentally stable. Factors contributed to higher absconding rate include overcrowding, lower number of staff. Some of the measures taken to overcome the high incidence of absconding were formulated including all new patients enrolled to the rehabilitation workshop should be escorted by ward staff and psychoeducation programme.
